NYSED Digital Equity Survey
In order to promote and plan strategies for digital and educational equity for all NYS students, the New York State Education Department is requiring districts to submit student digital access data. We are asking parents and guardians to help us fulfill this requirement and complete the Digital Survey form for each student in the family in grades Kindergarten – Grade 12. This survey will provide information on the state of our students' access to devices and internet services in their places of residence. The data you submit is confidential and will only be shared with NYSED.
If you have already completed the survey and your digital access has not changed, there is no action required. However, if your digital access has changed, please complete and submit the survey.
If you require any assistance or have questions, feel free to contact me by email (email@example.com) or phone (845-326-1190). To access this survey, log onto your Infinite Campus Parent Portal and click the Survey tab. If you are already in your IC Parent Portal, just click on the Survey tab.
