BMI School Survey Opt-Out Form

As part of the required school health examination, a student is weighed, and their height is
measured. These measurements are used to calculate the student’s Body Mass Index (BMI), which helps the doctor or nurse determine if the student’s weight is in a healthy range.

Recent changes to New York State Education Law require that a student’s BMI and weight
status group be included as part of the school health examination. A sample of school districts will be selected by the New York State Department of Health (NYSDOH) to take part in a
statewide survey.

If our school is selected, we will report only summary information about our students’ weight
status groups to the NYSDOH. No names or information that identifies individual students
will be included.

However, you may choose to exclude your child’s information from this survey.

The data collected by NYSDOH helps public health officials develop programs that support
healthier lifestyles for children across New York State.
